prescriber. It contains the signature, in ink, of the person who
wrote the prescription. If the prescription is for a controlled
substance such as a narcotic, the prescriber's signature,
branch of service, social security number, and name
(stamped, typed, or hand printed) must appear in this block
on the prescription form.

physician assistants, AMOSISTS, and physical therapists
