INITIATING HEALTH RECORDS
Health Records are prepared and maintained for all Army personnel. The HREC
is initiated as described below.
a. HRECs for Personnel Entering Active Duty. When an individual enters
active duty, A HREC is initiated by the officer who prepares DA Forms 2 and 2-1
(Personnel Qualification Record). ARNG and USAR members not entering on initial
active duty for training will have HRECs prepared by the custodian of the Military
Personnel Records Jacket (MPRJ). For example , ARNG or USAR AMEED officers
who receive direct appointment will have their health records prepared by the custodian
of MPRJ.
b. HRECs for Personnel Reentering Service. For personnel reentering
military service, a temporary HREC is prepared. The past HREC is acquired and the
documents in the temporary HREC are transferred to the past HREC. Requests for the
past HRECs are made by the military personnel officer of the first unit the person is
assigned to for training or other prolonged duty. Requests will not be made by
reception station personnel. Requests for past HRECs are sent to:

For ARNG, Health Records for personnel reentering the National Guard are requested
from the State Adjutant General of the state from which he was separated.
c. HRECs for Cadets of the U.S. Military Academy. Health Records are
initiated for cadets in the same manner as for active duty. These Health Records are
continued in use when cadets enter active duty.
d. Custody of HRECs. The HREC prepared for a person entering military
service is kept in his Military Personnel Records Jacket (MPRJ). It is not sent to a
Health or Dental Records custodian until the person arrives at a station where he will
remain 15 days or longer. Before his arrival at the station, the custodians of his MPRJ
retain custody of his HREC; however, they will send it immediately to a medical or
dental officer who requests it or treats the person. In the ARNG and USAR, the
custodian of the MPRJ is also the custodian of the Health Record.
