HEALTH RECORD FILE FOLDER
The forms to be prepared when a Health Record is initiated are: DA Form 3444-
Series folder, SF 600 (Health Record - Chronological Record of Medical Care), SF
601 (Health Record - Immunization Record) and also PHS 731, (International
Certificates of Vaccination), SF 603 (Health Record - Dental), SF 88 (Report of Medical
Examination), SF 93 (Report of Medical History), SF 545 (Laboratory Report Display)
and DA Form 5571, Master Problem List.
a. DA Form 3444-Series Folder. DA Form 3444 is the folder that will
contain the "Treatment Record" and is designed to be used for five different records.
It's use as a Health Record is covered in this lesson. The DA 3444-Series is also
designed to allow alphabetical or terminal digit filing in one folder. Because of this
design, records may be transferred from an MTF using alphabetical filing to one using
terminal digit filing without changing the folder. The Terminal Digit Filing System
(TDFS) was covered in lesson 1.
b. DA Forms 3444-Series Folders, Color Coded. These forms are color
coded and numbered as shown in figure 1-1. The color of the folder , as you learned in
lesson 1, is determined by the last two digits of the person's social security number
(SSN). These last two digits are called the "primary group."
c. Preparation of the DA Forms 3444-Series Folders. You learned to prepare
the DA Form 3444 in lesson 1. The steps are briefly reviewed in the following
paragraphs. As you learned in lesson 1, the DA Forms 3444 are 10 differently colored
folders and are prepared as follows for the Health Record:
(1) Select the correct colored folder. The color of the folder is based on the
last two digits (the primary group) of the SSN.
(2) Enter the person's name. Enter the last name, first name, and middle
initial along the upper left edge of the folder.
(3) Enter the person's SSN. Enter the last digit in the far right block on the
upper edge of the folder; enter the two digits of the secondary group in the two empty
blocks on the upper right corner of the folder to the left of the primary group; enter the
first five digits in the hyphenated blocks along the top of the folder to the left of the
secondary group.
(4) Identify type of record. Under "Type of Record," place an "X" in the box
to the left of "Health."
(5) Enter the member's blood type.
(6) Handwritten entries as described above are made in dark ink and boldly
printed.
