U. S. FIELD MEDICAL CARD (FMC)

4-1. INTRODUCTION
a. The U. S. Field Medical Card (FMC) is primarily a medical record used in
combat that provides a means to record data similar to that recorded on the Inpatient
Treatment Record Cover Sheet. The first medical person attending a casualty/patient
initiates the DD Form 1380 (U.S. Field Medical Card). The FMC goes with the casualty,
living or dead, to the MTF, or to the interment.
b. The purpose of the FMC is to provide the medical officer with information as
to the character of a patient's disease or injury and the treatment administered. It is a
source of information on the incidence, nature, and after effect of diseases and injuries.
This information can be used to aid in evaluating the quality and effectiveness of
measures to prevent illness or injury, maintain health, and provide care and treatment
for the ill and injured.
