Notes:
When application from disabled member for continuance on active duty is approved,
original DA Form 199 and medical board proceedings with enclosures, except medical
records and health record and similar supporting documents, are filed in member's official
military personnel file with copy of letter of approval. Medical records, health record, and
similar documents are returned with copy of letter of approval to medical treatment facility
that sent case to PEB.
