c. A profile containing one or more "3" designations signafies that the individual
has one or more medical condition(s) or physical defect(s) which require significant
limitations. The individual should receive assignments commensurate with his or her
physical capability for military duty.

RECORDING AND REPORTING OF INITIAL PHYSIC AL PROFILE
Individuals accepted for initial appointment, enlistment or induction in peacetime,
normally, are given a numerical physical profile designator of "1" or "2" (according to
instructions in AR 40-501). Initial physical profile designators are recorded on Standard
Form 88 (Report of Medical Examination) by the medical profiling officer at the time of the
initial appointment, enlistment, or induction medical examination.

PROFILING OFFICER
Commanders of Army MTFs are authorized to designate one or more physician(s),
dentist(s), optometrist(s), podiatrist(s), audiologist(s), nurse practitioner(s), and physician
assistant(s) as profiling officer. The commander must assure that those designated are
thoroughly familiar with the contents of profiling regulations specified in AR 40-501.

PHYSICAL PROFILE BOARDS (PPBD)
a. Physical profile boards are appointed by the MTF commander. They are usually
comprised of two qualified physical profiling officers; one must be a physician. A third
physical profiling officer is sometimes appointed in complicated or controversial cases or
when necessary to resolve disagreement between the two-member board.
b. Situations which require consideration of PPBD are specified in paragraph 9 -8,
AR 40-501.
c. Temporary Profiles. When, in the opinion of the profiling officer, the functional
capacity of an individual has changed to such an extent that it temporarily alters his ability
to perform duty, his profile may be temporarily revised. In this case, the profiling officer is
authorized to issue a temporary profile without referring the case to the PPBD or to the
PPBD approving authority. Temporary profiles written on DA Form 3349 will not exceed 3
months. Temporary profiles written on DD
Form 689, Sick Slip, will not exceed 30 days.
