environment. The Program for Persons with Disabilities is only for seriously disabled
persons who are dependents of active-duty members. You must apply and get
approved before TRICARE Standard can pay the costs of care.
d. Approval and Evaluation. Application is made by completing a CHAMPUS
Form 190a, "Request for Health Benefits Under the Program for Persons with
Disabilities," and mailed to the Director, TMA-WEST, Aurora, Colorado. DOD
Regulation 6010.8 -R outlines the specific type of information which must accompany
this application. The application is reviewed by the Director, TMA-WEST (or designee)
and approved or disapproved. Periodic reviews and reevaluations are required, in
accordance with DOD Regulation 6010.8 -R.
1-18. SERVICES AND CHARGES
a. Scope of Benefits. The Program for Persons with Disabilities provides
financial assistance toward the purchase of services and/or supplies necessary for the
following:
(1) Diagnosis.
(2) Inpatient, outpatient, and home treatment.
(3) Training, rehabilitation, and special education.
(4) Institutional care in private not-for-profit or public and state institutions
and facilities.
(5) Transportation, when appropriate, to and from such institutions and
facilities.
b. Cost Sharing. The sponsor is required to pay a portion of the costs for each
month in which the dependent receives benefits under the Program for Persons with
Disabilities. The amount the sponsor pays is based upon the pay grade. (See table
1-1.)
