b. Diagnostic Abbreviations. All abbreviations authorized for use on the Inpatient
Treatment Record (ITR) Cover Sheet may be used on the FMC. The ITR Cover Sheet
is discussed in Subcourse MDO753. Abbreviations may not be used for diagnostic
terminology, except for those listed below:

Cont W--Contused wound.
(3)
FC --Fracture (compound) open.
(4)
FCC--Fracture (compound) open comminuted.
(5)
FS--Fracture (simple) closed.
(6)
LW--Lacerated wound.

MW--Multiple wounds.
(8)
Pen W--Penetrating wound.
(9)
Perf W--Perforating wound.
(10)
SV--Severe.
(11)
SL--Slight.
c. Items of Entry. The instructions for preparing an FMC are discussed in the
following paragraphs. Figure 4-1 illustrates the front and back of a DD Form 1380.
NOTE: Although you are not required to initiate a DD Form 1380, you are required to
review the FMC for completeness and accuracy. Therefore, this lesson will require you
to know and enter the correct information on the FMC.

Enter patient's name (last name, first name, and middle initial).

Enter rank or grade.
NOTE: Use the abbreviations authorized by AR 310:50; CPT (Captain, 0-3); 1LT (First
Lieutenant, 0-2); CW4 (Chief Warrant Officer, W-4); WO1 (Warrant Officer, W-1); SGM
(Sergeant Major, E-9); MSG (Master Sergeant, E-8); SFC (Sergeant First Class, E-7);
SSG (Staff Sergeant, E-6); SGT (Sergeant, E-5); CPL or SPC (Corporal or Specialist,
E-4); PFC (Private First Class, E-3), and PV2 or PV1 (Private, E-2 or E-1).
