b. Preparation and Use of SF 601 and PHS Form 731 (Immunization
Records).
(1) An SF 601 (Health Record - Immunization Record) is prepared and
permanently kept for each person who has an OTR. It is put into the record when:
(a) The OTR is initiated;
(b) The patient next reports for immunization or sensitivity tests; or
(c) Reactions are noted.
(2) PHS Form 731 is prepared or posted when a patient reports to an MTF for
immunizations. Only the following identification information is entered on the form:
(a) The patient's name on the "Traveler's Name" line.
(b) The patient's address on the address line.
(c) The country of the individual's address on its appropriate line.
NOTE: The patient himself keeps this record.
(3) Those preparing SF 601 and PHS Form 731 will ensure that all entries are
recorded on both forms and that both forms are current with each other.
(4) International rules require that entries on PHS Form 731 for immunizations
against smallpox, yellow fever, and cholrea be authenticated. Each entry must show
the DOD Immunization Stamp and the signature of the medical officer or his chosen
representative. For other entries on the PHS Form 731 and all entries on SF 601, the
signature block may be stamped or typewritten and initialed by the medical officer.
c. Preparation of the OTR Folder for Patients Allergic to Medications. When
a patient is allergic to medications, the "Medical Condition" block is marked on the
outside cover of the DA Form 3444 and a DA Label 162 (Emergency Medical
Identification Symbol) affixed. This is done when DD Form 601, PHS 731, or DA Form
3365 is prepared.
d. Obstetrical Cases. A pregnancy diagnosis is entered on SF 600 for the OTR.
During the pregnancy, treatment is recorded in a separate OB record and usually kept
in the OB Clinic. This OB record is generally a manila folder in which all forms related
to the pregnancy are attached, i.e., lab work, prenatal history, progress notes, etc. After
the pregnancy, all forms related to it are filed, the following in the ITR (Inpatient
Treatment Record). When the records are filed, the following is entered on SF 600 for
the OTR: "Prenatal care records filed in ITR of (patient's name, FMP, and SSN,
(location of MTR), and (date)."
