(3) Chief, Patient Administration Division (PAD). The Chief of the PAD is
responsible for seeing that certain duties are performed. However, these duties are
usually delegated to other PAD personnel. When a death occurs in the hospital during
normal duty hours, the Chief of the PAD is notified. His representative, then, proceeds
immediately to the ward where the death occurred. The representative:
(a) Makes a thorough search of the bedside table and ward for money,
valuables, papers, clothing, and other effects belonging to the deceased. This search
is made in the presence of a ward nurse.
(b) Itemizes all money and valuables on DA Form 3696, Patient's
Deposit Record, signs the form, and obtains the signature of a witness.
(c) Lists all clothing and effects (other than money and valuables) on DA
Form 4160 (in duplicate), signs the form, and obtains the signature of a witness.
Dentures are included in the inventory.
(d) Turns over itemized money and valuables to the patient trust fund
accountable officer. Delivers clothing and personal effects, other than money and
valuables, to the Admissions and Dispositions Office/Baggage Room for safekeeping.
(e) Completes actions indicated in Section B of DA Form 3894, Hospital
Report of Death, (in accordance with AR 600-10, AR 40-2, and local SOP). Completes
Item 18 first. Enters "Report Received" under the type of action column, the hour, day,
month, and year along with his initials. This provides a record of when the DA Form
3894 was actually received in PAD. Hour, day, month, year, and initials must be
entered as an action is completed on each notification.
(f) Directs release of remains for movement to the morgue.
(g) Telephones the information to the Casualty Branch. (Local
procedures and AR 600-10 establish requirements for this information.)
(h) If an autopsy has been authorized in Item 21 of DA Form 3894, he
forwards the report to the Chief of Pathology for completion of Items 20 and 22 through
25.
(i) If an autopsy has or has not been authorized, he prepares Items 26
(Date) and 27 (Name, Grade, Branch) for authentication by the Chief, PAD, who must
sign Item 28.
