b. Handcarrying OTRs. If the patient (other than special category) requests, he
may handcarry his OTR to the next MTF. For children who are minors, the parent or
legal guardian may deliver their OTRs to the next MTF. When OTRs are handcarried,
the following procedures are followed:
(1) The patient signs for the OTR on DA Form 3705 (Receipt for Outpatient
Treatment/Dental Records0. For minor children, the parent or legal guardian will sign
the DA Form 3705. The "Address" block on DA Form 3705 must be completed at the
time the patient signs the form. Once signed, the DA Form 3705 is filed alphabetically
and kept until the retirement of that year's records. See figure 3-2.

(2) An adult's OTR will not be released to anyone other than the patient unless
a signed authorization is presented to the MTF. Any statement approving release to
another person will be acceptable if signed and dated by the patient. This statement is
attached to DA Form 3705.
