b. For OTRs and HRECs, put an identification label in the "Patient Identification"
block. When an addressograph data recorder is available, prepare the identification label
using the patient's recording card (PRC) (see figure 1-19). When a recorder is not
available, prepare the label to show the data given in lines 1 and 3 of the PRC; this data
must be shown in the format prescribed for the card (see figure 1-3). For ITRs, the
patient's admitting plate will be used to prepare the label and to stamp the folder. (See
Subcourse MD0752).

c. The patient's name is entered along the upper left edge of the folder (see figure
1-4). Data entered by hand will be written with a fiber-tipped pen unless otherwise
indicated.

d. Enter the last digit of the SSAN in the far right block on the upper edge of the
folder, then code the last digit of the patient's SSN on the folder. This is done by covering
the number that is the same as the last digit of the SSN, on the top and the right edge of the
folder, with half-inch black tape or blocking out with ink. (See figure 1-5).
