d. Artificial 11-digit numbers are given to patients not described in paragraph c
above and in table 1-1. These numbers are also given to patients who do not have a SSN
(e.g., a newborn infant of a dependent daughter). In the patient; the first patient of this kind
is 00001 and all later patients numbered consecutively. The last two groups of two digit
numbers are always 00 00. Thus, the first patient's number is 00 00001 00 00; the second
patient's 00 00002 00 00; and so forth.

USE OF DA FORM 3444-SERIES FOLDERS
a. The DA Form 3444-series and DA Form 8005-series are the only folders
authorized for filing Inpatient Treatment Records (TRs), Outpatient Treatment Records

(OTRs), Health Records (HRECs), Civilian Employee Medical Records (CEMRs), and
nuclear medicine files. Only DA Form 3444-series folders will be used for dental records,
ITRs, Alcohol and Drug Abuse Prevention and Control Program-Outpatient Medical
Record (ADAPC-OMRs), and CEMRs. DA Form 8055-series folders will be used only for
HRECs and OTRs. DA Form 8005-series folders will replace DA Form 3444-series
folders only when they have deteriorated or when beneficiaries are entering the system for
the first time. The DA Form 3443-series are the only authorized preservers for filing
nondental x-ray film.
b. The DA Form 3444-Series folders are designed to allow alphabetic or terminal
digit filing in one folder. Because of this design, records may be transferred from an MTF
using alphabetical filing to one using terminal digit filing without changing the folder.
c. The DA Form 3444-Series folders are also designed to allow either drawer or
open shelf filing. This is why information is entered on both the top and right edge of the
folder.

PREPARATION OF THE DA FORM 3444-SERIES
The preparation of the DA Form 3444-Series is governed by AR 40-66. The DA
Form 3444-Series are 10 different colored file folders. They are prepared as follows:
a. Select the correct colored file folder. The last two digits (the primary group) of
the patient's or the sponsor's SSN determines the color of the file folder. (See figures 1-1
and 1-2). Note the relationship of the primary group and the DA form number. The last
digit of the form number is the same as the first number of the primary group. These
numbers are preprinted on the file folder.
