PROCESSING AN INFORMAL INVESTIGATION
a. The Medical Treatment Facility (MTF) Commander, patient administrator, or
physician must initiate DA Form 2173, (shown in figure 3-2) and complete Section I in five
copies. This section shows the nature and extent of the injury or disease; in the case of
death, it will show the presumptive medical cause of death. Within seven days after the
incident, this form must be initiated and all copies forwarded to the unit commander.
b. Within 14 days after the incident, the unit commander must complete Section II of
DA Form 2173 to show duty status at the time of the incident and to explain the
circumstances surrounding it. The unit commander must attach supporting documents and
forward the original and 3 copies to the Special Court Martial Authority (SPCM).
c. Within 21 days after the incident, the SPCM must review the DA Form 2173 for
(1) If the SPCM approves, the form is properly annotated and the original and
two copies are forwarded to the final approving authority (unless the SPCM is the final
approving authority).
(2) If the SPCM disapproves, it directs a formal investigation.
d. Within 28 days after the incident, the final approving authority must review the
investigation for completeness and must review the required documents.
(1) If the SPCM approves, the form must be annotated as described in c(1)
above.
