5-24. NOTIFICATION
a. When the medical board recommends referral of the patient to a physical
evaluation board, the patient is furnished a copy of DA Pamphlet 360-506 (Disability
Separation) to provide information of the steps involved in processing.
b. The patient should not be told that he will be discharged or retired from the
service; nor should he be told the percentage of disability, or the line of duty findings until
final approval has been obtained from upper level authority.
5-25. ACTION FOLLOWING APPROVAL OF MEDICAL BOARD
RECOMMENDATION FOR PEB REFERRAL
Upon approval of a medical board recommendation for referral of a member's case
to PEB, the member's unit commander is notified and asked to inform the hospital
commander about pending disciplinary or personnel actions which would bar the member
from further physical disability processing. If further action is not barred, then records listed
below (as appropriate) are forwarded to the physical evaluation board by the most
expeditious means available.
a. Approved DA Form 3947 (Medical Board Proceedings) with enclosure 1 (SF
502, Narrative Summary) and enclosure 2 (SF 88, Report of Medical Examination, original
and 5 copies).SUBJECT: Request for Continuance on Active Duty
b. All clinical and medical records, including those received from MILPERCEN and
civilian sources, other uniformed services hospitals, and the Veterans' Administration.
X-rays are retained by the medical facility until requested by an adjudicative or review
body, then they are forwarded without delay. However, SF 519As (Radiographic Reports)
are forwarded with the clinical records.
c. DA Form 3444 or 8005 series (Health Record).
d. Reproduced copy of DA Forms 2 and 2-1 (Personnel Qualification Record, Parts
I and II for active-duty personnel), DA Form 66 (Officer Qualification Record), or DA Form
20 (Enlisted Qualification Record for reserve components personnel). A copy of Request
e. A statement outlining circumstances which preclude advancement to E-2 (if the
current grade is E-1).
f. A statement from the unit commander about any pending unfavorable action
which precludes processing. If such action is pending, a statement will be attached.
