j. Ensure that the patient completes the preprinted DD Form 2005 (Privacy Act
Statement - Health Care Records) on the inside of the folder. If the patient's DD Form
2005 is already completed, he does not need to complete a new one. It is important that
the patient read the statement before signing the form. This document informs the patient
of how the Army and Federal Government may use information in the medical record. (See
figure 1-11.)
