WWR DUE DATES FOLLOWING THE REPORTING PERIOD
The WWR is submitted by the MTF no later than 5 days after the close of the report
month. The Service reviews and consolidates the reports. If necessary, the Service works
with the MTF to correct it. The Service forwards the WWR toDoD(HA) no later than 15
days after the close of the reported month.

TYPES OF REPORTS (See Figure 1-3.)
a. Initial Report. As an MTF begins operations, an initial report covers a period
from the date the MTF became operational through the last day of the month (or to the date
the MTF closed during the same report period). An initial report is required when
MAJCOM jurisdiction of an MTF changes for the portion of the reporting month it operates
under an MAJCOM gaining jurisdiction. An explanation should be placed in the Remarks
section.
b. Monthly Report. If the MTF was operational for the entire report period, a
monthly report will be prepared and forwarded.
c. Corrected Report. Corrected reports will be prepared as required and will be
submitted electronically. If electronic reporting means are not available, manually prepared
correction reports will show entries only for the heading, corrected section, column(s),
line(s), and authentication. "Corrected" should be annotated above the heading in the
Report Type box of manual reports. All corrected reports are due to the Service NLT 45
days after the close of the reporting period.
d. Negative Report. If an MTF is operational but no workload is being generated,
a monthly negative report is required for control purposes. This type of report is used for
nonfixed MTFs that are activated for a contingency or mobilization, but have an interruption
in seeing patients for mission reasons. "Negative" should be annotated in the Report Type
box of manual reports. A statement to this effect will be made in the body of the report,
such as, "No patients were treated during this reporting period because...." The CHCS
does not support this type of report.
e. Final Report. The MTF will submit a report covering the period through the last
day the MTF was operational. If the MTF is included in a MEDCEN/MEDDAC other
consolidation, the closing date should be entered in the Remarks section of the
consolidated report. A final report is submitted by a MAJCOM losing jurisdiction of a
facility covering the period from the first of the month to the date of change.
